Position Description

As a Registered Nurse for our Adult Day program at AccessPoint RI, you will be responsible for providing comprehensive, professional medical services to all the individuals receiving day services. The RN needs to be clinically competent in the delivery of care to individuals utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. Another responsibility of the RN, is to effectively interact with individuals with their families as well as other clinical team members while maintaining standards of professional nursing.

As the Lead Registered Nurse Case Manager, you will be tasked with managing a caseload of adults with Developmental Disabilities living in our residential program and attends to their medical needs.  You are a member of a multidisciplinary team helping the individual and their family to manage their medical needs on a daily basis.  Responsibilities include training staff, ordering/organizing medications, accurate reviews of med passings, attending mandatory meetings, speaking with medical professional to coordiante care and answer staff questions to ensure safety.

Essential Duties and Responsibilities (RN):

  • Administers medications and treatments in accordance with all laws, regulations and rules of the state of Rhode Island and federal regulatory agencies.
  • Updates and maintains complete and accurate documentation of the individual's current health status and medical history. Includes medical status notes, annual nursing summaries, medical reviews and other related documentation.
  • Responsible to accurately transcribe physician's orders and to implement them in a timely manner.
  • Administers emergecy first aid accurately, when necessary.
  • Respect confidentiality and privacy, and communicates in a courteous and professional manner.
  • Attend scheduled nursing, clinical, core meetings, IEP's and ISP's.

Essential Duties and Responsibilities (Lead RN Case Manager):

  • Utilization of the nursing process by demonstrating appropriate assessment, planning, implementation and evaluation of the individual’s care. Make accurate and ongoing assessment of the individual’s status and respond appropriately.
  • Administer medications and treatments in accordance with all laws, regulations and rules of the state of Rhode Island and federal regulatory agencies (injections, dressings, etc.).
  • Update and maintain complete and accurate documentation of the individual’s current health status and medical history. Including medical status notes, annual nursing summaries, medical reviews and other related documentation.
  • Responsible to correctly transcribe and implement physician’s orders in a timely manner.
  • Administers emergency first iad immediately and accurately, when needed.
  • Train individuals and staff as necessary, providing appropriate health and wellness information.
  • Demonstrate ability and appropriate technical skills when administering treatments and procedures in accordance with physician’s orders and agencies policy and procedure. Provides direct patient care when necessary, including emergency first-aid.
  • Respects confidentiality and privacy and communicates in a courteous and professional manner.
  • Consistently works in a positive and cooperative manner with the nursing team, clinical team members, agency employees, family members, physicians, and other community health providers.
  • Provides nursing coverage to other programs when needed.
  • Follows on-call protocol for evening and weekend nursing coverage.
  •  Attends scheduled monthly meetings i.e. nursing, clinical, house meetings, core meetings and IP’s.

Minimum Qualifications:

  • Ability to pass background clearances.
  • Regular access to an insured vehicle.
  • A minimum of 2 years experience working with individuals with Developmental Disabilities is preferred.
