Position Description

Summary:

Reports to the Clinical Nurse Manager, Home Infusion. Responsible for delivering infusion nursing services to patients as prescribed by their healthcare provider. Assesses patients, develops care plans, and educates them about the infusion process, their prescribed medications, and the equipment needed for treatment. Collaborates with the prescribing provider to manage the patient's care plan, administer medications, monitor patient responses, and communicate any changes in the patient’s condition. Coordinates with other members of the healthcare team to ensure cohesive, coordinated care, and actively participates in educational initiatives, quality assurance efforts, and patient satisfaction programs.

Responsibilities:

Coordinates receipt of patient care referrals to program. Obtains intake information necessary to establish the patient for treatment including, but not limited to, patient demographics, insurance information, past medical and surgical history, allergies, height, and weight.

Communicates patient information, both verbally and electronically, to appropriate departments and outside agencies, as necessary.

Performs tasks necessary to initiate and administer prescribed treatment(s), including, but not limited to, scheduling appointments, ordering medication and supplies, and communicating with other stakeholders, as necessary.

Ensures that the consent form has been signed and is on file and verifies that prior authorization for the medication has been obtained and is in place before administering the treatment.

Ensures the required procedures and labs have been completed and results are reviewed by the prescriber prior to medication administration.

Performs an initial physical assessment of the patient, including the venous system.

Administers prescribed medication, evaluates the patient’s response, and advises the prescriber of any adverse events or concerning changes in patient’s condition. Reports any adverse reactions as required including the Safety Net system.

Develops and maintains a plan of care based on the patient's current needs in relation to the prescribed treatment.

Provides, coordinates, and documents education for patients, families, and/or caregivers on managing the patient’s health condition(s), including guidance on health promotion and rehabilitation.

Educates the patient, family, and caregiver about the prescribed treatment and provides instructions on the use and maintenance of any vascular access devices in place.

Collaborates regularly with the patient, family, caregiver, prescriber, and other healthcare team members to integrate, assess, and maintain continuity of the plan of care.

Prepares medications for infusion using aseptic technique, in accordance with manufacturer recommendations, established policy and procedures, and standards of clinical care. Completely and clearly labels the medication.

Performs peripheral venipuncture and midline insertions in accordance with established policies, procedures, and clinical care standards.

Provides care and maintenance for vascular access devices, including their removal, following established policies, procedures, and clinical care standards.

Ensures all equipment, including infusion pumps are maintained, inspected per protocol and in accordance with accrediting body standards.

Collaborates with colleagues to ensure the patient's response to treatment is reported to the prescriber.

Monitors professional, medical, and technological developments and incorporates these into clinical nursing practice.

Participates in educational activities based on learning needs and individual abilities, including but not limited to conferences, seminars, and the orientation of new staff.

Participates in clinic and departmental quality assurance activities, including monitoring and evaluating care, validating nursing and ambulatory care standards, and ensuring adherence to established policies, procedures, and accrediting body standards.

Serves on committees, participates in organizational activities involving professional nursing practice and patient care standards.

Participates in clinic and department level collaborative activities, including, but not limited to, joint case presentations and grand rounds.

Performs other duties as assigned.

Other information:

BASIC KNOWLEDGE:

Graduate of an accredited School of Nursing. Bachelor of Science in Nursing from an accredited College of Nursing preferred.

Maintains current unrestricted licensure to practice as a Professional (Registered) Nurse (RN) in the State of Rhode Island or another state that participates in the Nurse Licensure Compact (NLC).

Must possess and maintain current American Heart Association (AHA) Cardiopulmonary Resuscitation (CPR) certification within 60 days of hire.

Demonstrates knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages, and cultural patterns in each step of the care process.

 

EXPERIENCE:

Minimum of 1 year experience as a practicing RN, preferably gained in an infusion center or home healthcare setting.

Demonstrated high level of oral and written communication skills.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Work may require extended periods of time spent standing, walking, bending, and/or stooping.

Exposed to all patient elements, including patient home environments.

Requires the visual and manual dexterity to operate a computer.

Requires travel between sites and reliable personal transportation.

INDEPENDENT ACTION:

Performs independently within department policies and practices

None.
